With a budget of around £270m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

We are currently looking for enthusiastic and highly motivated health care professionals to join our highly professional and motivated directorate to support the development and delivery of an Enhanced Care service.

The Enhanced Care Worker will be responsible for providing 1:1 care and support to patients presenting with complex health and social needs, under the guidance of the Enhanced Care Coordinator.

They will work collaboratively with multi-disciplinary team members to ensure a high and consistent standard of enhanced care is delivered to individual patients across the Trust to optimise patient outcomes.

These posts are firmly based in clinical practice and involves working directly with patients, carers and multidisciplinary teams.
