We are seeking a full time, Band 7 Health Improvement Manager who is highly motivated, inquisitive and creative problem solver with excellent communication and influencing skills, to join our One You Hounslow and One You Merton Health and Wellbeing Services working across Hounslow and Merton Boroughs. The successful candidate will be the operational lead in the delivery of Health and Wellbeing services, ensuring the development and implementations of interventions, to meet the local targets for the improvement of health and the reduction of inequalities present within the London Boroughs of Hounslow and Merton.

The successful candidate will bring together the key partners and stakeholders to deliver an effective and efficient service achieving the required outcomes that will demonstrate a measure able and incremental change; whilst supporting the borough's prevention agenda.

You will have a track record in delivering change in a range of health and wellbeing settings, and be able to quickly develop credible, productive relationships with key stakeholders. Candidates should be able to demonstrate good knowledge and application of quality improvement methodologies and project management skills, relevant to health care systems.
