This is an exciting opportunity to join the Trafford Team during service realignment and departmental integration.

This post holder will play a key role in supporting and managing patient pathways and aiding the Specialties in ensuring timely treatment for Trafford patients.

The post holder will be required to ensure that all data relating to the monitoring of the RTT pathway for patients is collated and processed in accordance with national and local timescales for submission of data. The post holder will also provide timely information to Trust Managers to ensure that national targets are not breached. Working under the direction of the DDM, the post holder will work closely with Administrative, Clinical staff, Finance and Business Analysts to ensure that the correct procedures are recorded on PAS.

The post holder will need to have thorough knowledge of and provide local expertise on the national RTT pathway target. Develop an in depth understanding of the various patient pathways associated within each specialty. Deliver a robust system for validating and analysing data, and tracking patients along the RTT week pathway using the centralized HIVE system. Flag up the barriers and bottlenecks in clinical pathways that will affect the achievement of the target. Work with Clinical, Administrative and Managerial colleagues to redesign pathways to deliver smooth, fast pathways of care. Maintain systems and processes to guarantee an efficient and effective service that supports the delivery of activity and performance targets.
